How are Amazon FBA fees worked out?

Amazon takes a referral fee, a percentage of the selling price set by the category, and an FBA fulfillment fee, a fixed amount set by the package's size tier, its shipping weight and the price band, plus a fuel and logistics surcharge and storage by the cubic foot. The Amazon FBA Calculator works out all of it from the package's measurements.
